A patient-centered medical home is a trusting partnership between a doctor-led healthcare team and an informed patient. Family Health Care of Siouxland clinics are dedicated to meet the health care needs of this delivery model that puts you and your family, if you wish, at the center of your care. This care will be comprehensive including physical, dental and behavioral health to meet your personal needs. We will partner with you, various agencies and services throughout the surrounding area to match your personal needs by means of information technology, registries and health information exchanges.
As your primary care provider you can expect:
- FHC providers and staff to provide clear communication and evidence based health care that supports the goals and decisions you make regarding your healthcare conditions and treatment options
- Your team to answer questions or listen to concerns to help you better understand your health care needs and goals
- To be connected with other medical experts and/or additional services as needed
- Timely access to health care with a doctor on call 24 hours a day, 7 days a week and three conveniently located Urgent Care clinics
- Respect for you as an individual and your privacy
- To be kept up to date on your vaccinations and preventative screening tests
- To be notified of test results in a timely manner
- Care that is culturally competent and matched to your needs
As our patient we trust you to:
- Know you are a full partner in your care
- Make of list of your most important questions and concerns to discuss with your provider
- Bring a current medication list to every appointment include all over the counter medications and supplements you take
- Bring a list of any other providers that are part of your care and the reason why you see them
- Ask any other providers that are part of your care to send us their reports
- Follow the recommendations of your health care providers to maintain optimal outcomes for your health and well being
- Keep your appointments as scheduled or call in advance to change
- Provide feedback to help us improve our services
